Intake Questionnaire

I have been provided with Chigbu & Co’s Intake Questionnaire to complete prior to my consultation. At the consultation, the attorney will rely on the answers to this questionnaire as well as oral comments. I understand that the initial consultation fee is $550.00 for the first hour. The consultation fee will be credited towards the total fee, should I retain Chigbu & Co, LLP within 30 days of this consultation. I understand that Chigbu & Co, LLP may waive the consultation fee at its discretion, in writing, provided it has assurance that its services would be retained or the person requesting consultation is an existing or past client. I further understand that the Initial consultation fee does not apply to certain types of referrals. In such cases, Chigbu & Co, LLP will discuss and set the consultation fee amount prior to the consultation.

The purpose of the consultation is to provide Chigbu & Co, LLP with sufficient information to analyze the legal question or issue, including the timing and cost for achieving the results sought. I further understand that occasionally, the problem presented requires additional time after the consultation meeting.This may include the need for research, or further inquiry to investigate and evaluate the case. I understand that should I elect not to retain Chigbu & Co, LLP or if Chigbu & Co, LLP decides not to accept my case, the firm or any attorney employed by the firm, is under no obligation what so ever to represent me or any of the parties mentioned in this questionnaire. This is very important because I understand there may be time deadlines associated with the objectives sought and benefits that may be obtained.
